Overview
The Quality Department staff offers in person Error Prevention classes. Error Prevention establishes safety behavior expectations for all staff and educates to specific tools staff can use that have been proven to decrease harm. This class is strongly recommended and contains important information as we move toward a stronger safety culture of zero harm.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Incorporate into practice knowledge of Solutions for Patient Safety Framework and how Phoenix Children's incorporates these principles.
  2. Incorporate into practice a safety culture and foster a high reliability organization.
  3. Define a Serious Safety Event (SSE) and take appropriate steps to mitigate a Serious Safety Event when personally involved.
  4. Define and follow the Behavioral Expectations for all employees at Phoenix Children's.
  5. Identify and utilize Error Prevention tools to meet the Behavioral Expectations. (a. Utilize the Error Prevention tools to assist in decreasing Serious Safety Events by 10 percent by June 2025, b. Utilize the Error Prevention tools to assist in improving Central Line Associated Bloodstream Infections by 10 percent by June 2025.)
  6. Report safety events to improve our safety culture.
  7. Incorporate into practice knowledge of how trending safety events leads to process improvement and the basic methodologies Phoenix Children’s uses for improvement.
